Areas of Assistance & Guidelines

  • Funeral expenses related to death of employee or employee’s immediate family member*.
  • Travel assistance to attend the funeral of an immediate family member* or to visit an immediate family member* that is suffering from a life-threatening illness or injury and death is imminent.
  • Damage or Loss of home due to a fire or natural disaster.
  • Temporary disaster displacement of an employee due to a classified state or federal disaster.
  • Extraordinary expenses (other than medical, healthcare expenses, or lost wages) arising when an employee or their immediate family member* has a life-threatening or unexpected acute illness or injury that requires extended hospitalization and/or other extensive medical treatments. The immediate family member must live with employee.  
  • Uninhabitable or condemned rental housing employee’s home that they are renting is condemned and/or classified as uninhabitable by local authorities due to unsafe living conditions. 

*Immediate family member is defined as spouse, domestic partner, parent (including in-laws), child, grandchild, grandparent, or sibling. The definition of child includes biological, legally adopted, foster children, stepchildren, and legal wards.

Guidelines:

1. Eligible individuals include employees (defined as regular full and part-time) who are experiencing a financial hardship due to circumstances beyond the employee’s control related to one of the areas of assistance listed above.

2. Employees requesting assistance must complete and sign an application and provide supporting documentation required by the Foundation.

3. Applications must be filled out completely and submitted with appropriate supporting documentation (listed on application). Incomplete applications will not be approved.

4. To be fair and objective, the selection committee will consider each request on a “blind basis” and base its decision solely on the need.

5. An employee may not apply to or receive assistance from the Employee Assistance Foundation more than twice in a 12-month period.

6. Employees must request assistance within 60 days of loss and are employed with DG at the time of the event.
